Crystal Lake Ophthalmology Associates was established in January 2000, to coincide with the start of the New Millennium. As such, it has been our goal and privilege to provide state-of-the-art, high-tech yet personal and compassionate diagnostic medical, laser and surgical eye care services to the population of Crystal Lake, IL and the greater surrounding McHenry Co area.

Edward G. Dolezal, MD is an experienced eye physician and surgeon who has been practicing ophthalmology in the Crystal Lake and greater McHenry County, IL area for nearly twenty years. A native of La Grange Park, IL, he completed his undergraduate studies at the University of Illinois in Champaign-Urbana, graduating Phi Beta Kappa and with Bronze Tablet Honors, the university’s highest academic award, earning a Bachelor of Science Degree in the Life Sciences.
